Associate Community Health Worker (CHW) BLS Certified Waynesville, MO $34,499 - $46,099 a year Full Time Join a dedicated healthcare team in the Midwest region as an Associate Community Health Worker (CHW), where your role will be vital in connecting patients with essential community services. Reporting to the Director of Care Management, you will support individuals and families by providing outreach, education, and assistance navigating social and health-related resources, with a special focus on high-risk populations. Key Responsibilities Build and maintain strong relationships with community partners, patients, and their families to promote healthy behaviors. Assist clients in accessing social and health services by assessing needs and making appropriate referrals. Develop and implement processes to evaluate Social Determinants of Health across various settings. Participate in community events such as health fairs to raise awareness and provide resource information. Advocate for individual and community health needs, acting as a liaison between healthcare providers and community organizations. Create and manage referral and follow-up processes to ensure patient needs are met. Distribute educational materials tailored to the target population. Education An associate degree in public health, social work, psychology, sociology, or related fields such as communications, education, or business is required. Experience Previous experience with social services or public health systems is preferred but not mandatory. Certifications and Licenses Basic Life Support (BLS) certification must be obtained within the first 90 days of employment; training is provided. Certification as a Community Health Worker is required within the first six months. Mental and Physical Requirements This role requires sustained mental focus with frequent interruptions, light lifting up to 15 lbs, and the ability to stand, sit, walk, read, and communicate effectively. Working Conditions Typical office environment with some noise and distractions, potential eye strain from computer use, attendance at public meetings, and possible exposure to infectious diseases during patient interactions.
