Associate Community Health Worker (CHW)Join a dedicated healthcare team in the Midwest region as an Associate Community Health Worker (CHW), where your role will be vital in connecting patients with essential community services. Reporting to the Director of Care Management, you will engage with community members to provide outreach, support, and valuable information, focusing especially on high-risk populations.
Key Responsibilities- Build and maintain strong relationships with community partners, patients, and families to promote healthy behaviors.
- Assist individuals, families, and groups in accessing social and health-related services.
- Evaluate client needs and refer them to appropriate community resources, developing processes to assess Social Determinants of Health across various settings.
- Participate in community events such as health fairs to stay informed about local issues and share resource information.
- Advocate for the health needs of individuals and the broader community.
- Serve as a liaison between healthcare providers and community organizations.
- Develop and manage referral and follow-up processes to address patient needs effectively.
- Distribute educational materials tailored to the target population.
- Perform additional duties as needed to support organizational goals.
An associate degree in any field is required.
ExperiencePrevious experience with social services or public health systems is preferred.
Certifications and Licenses- Basic Life Support (BLS) certification must be obtained within the first 90 days of employment; training is provided.
- Certification as a Community Health Worker must be earned within the first six months of employment.
This role requires considerable mental focus with frequent interruptions. Light lifting up to 15 lbs., standing, sitting, walking, reading, and speaking are part of daily activities.
Working ConditionsTypical office environment with some noise and distractions. Possible eye strain from computer use. Attendance at public meetings is required. There may be exposure to infectious or communicable diseases while interacting with patients in various settings.
