Job Title: Community Health Worker Location: Remote Duration: 6 Months of contract Shift/Schedule: Starting shift 9:30-6pm Monday - Friday Once full business hours of 9am-8pm are open shift would be 12-8 Monday – Thursday and 9:30-6 on Fridays The position provides support to the IL Medicaid Care Coordination team by outreaching to IL Medicaid members to complete their initial health risk screening and completing door knocks when necessary to contact hard to reach members. Telephonic, Work from home position in Chicagoland area in case they need to go onsite for a meeting (seldom) JOB REQUIREMENTS: Bachelor Degree OR 4 years health care, social services or community advocacy experience, Knowledge of medical terminology Experience coordinating member related needs, providing assistance to members, and analyzing member needs PC proficiency including Microsoft Office applications Customer service skills Preferred requirements Community Health Worker (CHW) certification from Client approved college Bachelor's degree in Social Work
