Minimum Requirements
- Master's Degree in Speech-Language Pathology from an accredited educational program.
- Certificate of Clinical Competence (CCC) in Speech-Language Pathology from ASHA, or CCC-eligible and holds current state license as applicable.
- Active Illinois state license to practice required to start the assignment.
- Proof of current CPR certification required.
- Current driver's license, reliable transportation, and vehicle insurance required.
- Minimum 1 year clinical experience in Speech-Language Pathology required.
- Experience with home health environment preferred.
- Experience with HCHB, OASIS, and home health documentation systems required.
- Ability to perform patient assessments, develop Plans of Care, recommend assistive devices, provide caregiver education, and document progress.
- Ability to work a flexible schedule, travel locally, and perform physical tasks such as lifting, bending, and prolonged standing.
- Must read, write, and speak fluent English.
Assignment Details
- Facility Type: Home Health.
- Assignment Type: Per Diem / Temporary Contractor.
- Float Requirement: Candidate may be required to float to other locations within the region.
- On-call: On-call required if requested; on-call needs are subject to change based on staff and branch needs.
- Expected Weekly Productivity: 30 units; typical weekly target 6 units per week.
- Required Systems Experience: HCHB and OASIS documentation experience required.
