Schedule / Job Type

Contract | Days | 3x8-Hour Shifts (Wednesday–Friday, 8:30 AM–5:00 PM) | Onsite / Field-Based Home Health

Key Responsibilities

• Provide skilled speech-language pathology services to home health patients in a 1:1 care setting
• Conduct patient evaluations and deliver therapeutic interventions across an assigned caseload of up to 6 patients per day
• Complete required documentation using EPIC and OASIS charting systems accurately and in a timely manner
• Manage a geographically grouped caseload within the service area, with occasional travel up to 50+ miles required
• Meet standard productivity expectations of 30 productivity points per shift

Required Qualifications

• Active or pending Speech Language Pathologist license in the state of Oregon
• Minimum 1 year of skilled home health experience
• OASIS documentation experience required
• Current Basic Cardiac Life Support (BCLS) certification
• COVID-19 primary vaccination series plus one booster required
• Reliable personal transportation required

Preferred Experience / Skills

• Experience with EPIC electronic health records system
• Familiarity with home health productivity standards and point-based expectations
