Key Responsibilities
- Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans and goals in accordance with patient needs and physician direction.
- Collaborate with the interdisciplinary team to address barriers to discharge, including equipment trialing and ordering, patient and family training, home exercise program issuance, and coordination of continuing services.
- Manage individual, concurrent, and group treatment sessions effectively, demonstrating flexibility in caseload management and ensuring continuity of the plan of care throughout the patient's length of stay.
- Complete accurate and timely medical record documentation including daily notes, goal writing, functional quality indicator assessments, letters of medical necessity, discharge documentation, and charge entry in compliance with applicable state, federal, and intermediary guidelines.
- Participate in student programs as assigned, providing supervision, developing learning objectives, and completing student evaluations.
- Adhere to discipline-specific state practice acts and regulatory requirements, maintaining professional licensure in good standing and fulfilling all continuing education requirements.
Required Qualifications
- Active and unrestricted Speech Language Pathologist licensure in the state of New Jersey.
- Ability to perform comprehensive patient evaluations and develop clinically reasoned treatment plans focused on functional outcomes and discharge planning.
- Proficiency in completing medical record documentation in compliance with state, federal, and payer guidelines.
- Knowledge of and adherence to discipline-specific professional codes of conduct and ethical standards.
- Demonstrated commitment to patient and employee safety practices.
Preferred Experience / Skills
- Experience working within an inpatient or acute rehabilitation setting.
- Familiarity with interdisciplinary team collaboration and discharge planning processes.
- Active involvement in continuing education, professional organizations, or community advocacy efforts.
